CARE TRANSITION NAVIGATOR
The Care Transition Navigator is a clinical liaison between health care providers and recently discharged hospital patients to ensure continuity of care for patients’ transitioning from hospital to home. Receives patient referrals for services from hospital Discharge Planners, directly communicating with and assessing the discharged patient to reduce the patient’s possible re-admission to the facility. The Care Transition Navigator develops relationships in the community to continually provide clients with needed referrals and services.
• Works with hospital Discharge Planner and meets with patients before being released from the hospital.
• Explains the Care Navigation Program and services provided by SeniorServ.
• Visits discharged patient within 24 hours of going home.
• Ensures the patient is aware of and attends follow-up medical appointments
• Ensures that patient is receiving home health equipment and other services as stipulated in the discharge plan
• Ensures that patient has all discharge medication and identifies needs for education.
• Refers patient to Home Health or Program RN as needed.
• Conducts fall risk assessment and recommends safety measures to reduce falls.
• Contacts primary care physician if patient appears to have difficulty in following discharge plan.
• Documents any barriers or concerns patient is having transitioning from the hospital to home and following discharge plan.
Bachelors degree in Social Work, Social Services, Gerontology or an LVN license with three years experience working in a hospital setting. Must have excellent communication and interpersonal skills. Good problem solving skills required. Ability to identify client and family needs and connect client to appropriate resources. Must have knowledge of community resources and regulations regarding benefits available to participants. Proficient in computer-based skills and have working knowledge of general office computer systems, internet, e-mail and desktop navigation. Weekend and evening work required. Must pass post-offer physical examination. Must have a valid California Driver’s License and automobile insurance. Will be traveling in Orange County and surrounding counties. Bi-lingual preferred. Background check required.
Anaheim Adult Day Health Care Center
Immediate Opening - 29 hours/wk
MASTER OF SOCIAL WORK
The Medical Social Worker completes assessments of psychosocial needs of participants and develops client care plans. Completes home evaluations and prepares reports on recommendations. Negotiates participation agreement with clients and families. Provides counseling to participants and their families. Makes referrals to local social service agencies for the benefit of clients and families. Completes discharge planning reports for participants.
Must have MSW from an accredited school of Social Work and at least one year experience and at least one year working with older adults. Must have CPR and first aid certification, possess valid California driver’s license and provide proof of automobile insurance. Must pass post offer physical.
SITE MANAGER LUNCH PROGRAM
FOUNTAIN VALLEY SENIOR CENTER
Part-time Tuesday & Thursday 5 hours per day $10.46/hr
The Site Manager is responsible for planning, coordinating and managing the Lunch Meal Program Dining Site. Responsibilities include meal preparation, administrative services, kitchen management, and supervising volunteers and support staff while meals are being served. The Site Manager initiates educational, social and entertainment programs at the dining site and works closely with seniors to ensure that the lunch meal program meets the needs of the participants.
The position requires skills in food preparation, event planning and administration. Previous experience in handling and/or serving food a plus. Management experience preferred. ServSafe certificate required within 60 days of employment. Must pass a post offer physical, be able to lift up to 45 pounds and stand for extended periods of time.

Immediate Opening/Full Time
The Registered Nurse provides skilled nursing services to the Adult Day Health Care Center participants. Provides health education instruction to participants and families. Completes initial and continuing assessment and develops a written care plan. Administers medications and monitors self-administration of medications by participants. Acts as liaison to the participant's personal physician. Supervises LVN and program aides.
Candidate must be a graduate of an accredited nursing school with at least one year of nursing experience. Currently licensed and registered in California by the California Board of Registered Nursing. Experience working with the disabled and/or the elderly. Must pass a post-offer physical examination, including TB clearance; and bilingual Spanish a plus.
SITE KITCHEN SUPERVISOR – PART TIME
SENIOR LUNCH PROGRAM
HUNTINGTON BEACH SENIOR CENTER
Monday – Friday 4 hours per day-$8.44/hr
The Site Kitchen Supervisor works out of the Huntington Beach Senior Center and is responsible for all kitchen activities relating to serving of noon meals to seniors at the Center. This will include re-heating food items and arranging all other food items for the senior lunch. The Kitchen Supervisor will be responsible for serving meals to seniors and for cleaning all surfaces, equipment and utensils after the meal. The Kitchen Supervisor orders, receives and stores food and supplies, follows menus to complete meal preparation and maintains the kitchen in a clean and sanitary condition. Previous experience in ordering, handling and/or serving food desired. Must pass a post offer physical and be able to lift up to 25 pounds.
Program Aide at Adult Day Care Center in Buena Park
Monday – Friday 30 hours/week $8.50/hr or DOE
Under the general direction of the Program Manager, the Program Aide assists with educational and social programs.
- Assists in food services to participants.
- Assists in kitchen duties.
- Assists with daily group and individual activities, such as exercise, games and crafts.
- Will accompany participants on field trips, walks, etc.
- Responsible for housekeeping responsibilities.
- Performs other tasks as requested.
Must have 2-3 years experience working with the elderly population. Experience in providing care giving services which include assisting elderly or disabled adult in walking, transferring and toileting activities. Experience in providing structured activity programs to the elderly. Must pass a post-offer drug test, physical examination and obtain TB clearance. Must have current certification in CPR and first aid training. Must be able to lift 45 pounds.
