Case Management

If you or a loved one needs help obtaining and understanding services that might be available to them, professional case managers are experts at navigating the confusing system.

Services

Case Managers make in-home visits, assess social and health care needs, and work with clients and family members to write out a plan of care. They will listen to concerns, identify critical needs, and provide available resources and information. They also follow up with clients and family members while continuing to support their needs.

  • Legal matters
  • Home safety assessments and modification/equipment
  • Friendly visitors; advocacy, and more

Case Management services allow frail seniors to continue living at home independently or with their families and loved ones while avoiding premature institutional care.

Eligibility

Case Management services are offered to individuals:

  • 60 years of age or older who are unable to carry on normal day to day activities
  • Who don't have anyone to assist him/her, and/or is in need of a range of services to maintain an independent lifestyle
  • Residing in Community SeniorServ's service area

Referrals for Service

Referrals may be made by any concerned individual, family member, neighbor, social workers, physicians, hospital discharge planners, or any other person by calling the Home Services Department at (714) 220-0224, Monday through Friday, 8:00am-4:00pm.

After a referral has been received, a Case Manager makes a home visit with the interested individual within 14 days. A thorough, systematic assessment of an individual's functional skills, cognitive status, limitations, needs, strengths, abilities and available personal and community resources is conducted during the home visit.
